Informed Consent & Terms – SEVEE CARE PVT. LTD. By selecting and using the services of SEVEE CARE PVT. LTD., I, [Login ID], acknowledge and give informed consent to the following: Terminology For the purpose of this agreement, the terms “Therapist,” “Counselor,” and “Consultant” are used interchangeably and refer to the service provider associated with SEVEE CARE PVT. LTD. For consistency, the term “Therapist” will be used throughout. Emergency Services I understand SEVEE CARE does not provide emergency services. In case of emergency, I will contact my treating doctor or visit the nearest hospital. Appointments & Sessions Sessions are by appointment only and conducted via video call. Each session is 45 minutes; sessions extending beyond 60 minutes will incur additional fees. If I am late, missed time will not be compensated. In case of absence, the Therapist will wait 15 minutes, post which I will be marked as a no-show. Communication outside sessions is limited to scheduling matters. Rescheduling Policy I agree to reschedule at least 6 hours in advance. Changes within 6 hours or no-shows will incur the full session fee. I take responsibility for tracking the validity of my purchased service. Therapist & Time Selection I understand that Therapist availability is based on the calendar. If time is my priority, I will select a Therapist available at my preferred time. If I prefer a specific Therapist, I will book according to their available slots. Manage Bookings I will manage my bookings, subscriptions, and appointments through my login and stay updated with policies. Confidentiality & Consent I consent to confidential session notes being maintained for professional purposes. I will not record sessions without written permission, and the same applies to the Therapist. Third-Party Communication I understand that no information will be shared with any third party without my explicit consent. Consultant Liability I acknowledge that my Therapist is a consultant with SEVEE CARE PVT. LTD., and the company is not legally responsible for individual practices. Client Responsibility I acknowledge that my mental and emotional well-being is my responsibility. SEVEE CARE provides guidance, not guaranteed outcomes, and is not liable for any personal loss or decisions.
